Questionnaires & Inventories

A comprehensive psychoeducational evaluation always requires collecting information about the individual’s cognitive, academic, behavioral, social, and emotional functioning across various settings. This is why we generally require provision of background information and additional reporting from more than one informant. We typically request both parent(s) and at least one teacher to complete questionnaires and inventories rating the child’s behavioral, social, and emotional functioning in their respective context. We also ask the child, adolescent, or young adult to complete self-report questionnaires to supplement these reports and our observations.
